UPDATE

The server has been reset to repeat invitations to enroll every three days, whereas before it was set to send every ten days.

Without regard to whether you were enrolled in the previous patient portal, you WILL need to re-register for security of you your patient information.

Confirmation of your identity will be your birth date and the zip code we have on file for you. If I do not have your email address, obviously the server will not send you an invitation. If you have not provided it to our office, send me a message and I will load it so that you get the invitation.

Again, for security reasons, THERE IS NOT A WAY FOR YOU TO SELF REGISTER. YOU MUST REGISTER BY RESPONDING TO THE EMAIL.

If you have an email address on file with us and you have not received an invitation, you may need to check your spam folder. If there is not a message in there from cleburnefma.com, send me an email at dnmoore@cleburnefma.com and I'll look into it.
